An overview of problems encountered in nursing: A first step toward an NMDS

In 2012, Switzerland will adopt a system for hospital funding called SwissDRG, based on the German Diagnosis Related Group model. This method of funding takes little account of nursing, since the grouping is based on diagnoses and treatments. Objective: This study conducted at Lausanne University Hospital seeks to introduce nursing data into medical statistics under the form of diagnosis. Method: Based on a web questionnaire, this study interviewed 293 nursing, clinical, and administrative managers. Based on their experience, they had to selected ten care problems from a list of thirty and sort them depending on their impact on workload and the length of stay of the patient. Results: Ninety-five responses from the majority of the clinical specialties at Lausanne University Hospital were analyzed. From this, a list of fifteen nursing problems was established. The use of ridit scoring shows that there is a difference between the subgroups, determined by gender and length of service. Conclusion: This study showed that the use of web questionnaires is a method that should be continued. It also shows that by using ridit scoring, it is possible to analyze the differences in responses between groups, even if the sample is small. This study must now be extended to pediatrics and obstetrics.
